एड्स का अर्थशास्त्र और राजनीति

एड्स कोई बीमारी नहीं है. बल्कि यह सिर्फ एक स्थिति है. एक ऎसी स्थिति जिसमें शरीर का प्रतिरक्षा तंत्र धीरे-धीरे नष्ट हो जाता है. यह संक्रामक है और फैलता है शरीर के कुछ खास द्रवों के माध्यम से. यह बात आज देश का बच्चा जानता है. इसी साल जून तक बताया जाता रहा है कि हमारे देश के हर सौ में एक आदमी एचआईवी पोजिटिव है. इस बात पर भरोसा करें तो हमारे आसपास ऐसे लोगों की भीड़ होनी चाहिए, जो नहीं है. फिर भी संयम में विश्वास और उस पर अमल करने वाले देश के नागरिकों को हर चौराहे पर बोर्ड लगाकर बताया जा रहा है कि वह चलें कंडोम के साथ. यानी स्वयं अपने चरित्र-अपने ईमान पर भरोसा करना छोड़ दें. गोया पूरा देश राजनेता हो गया हो. आखिर ऐसा क्यों किया जा रहा है? मलेरिया, डायरिया, टीबी, कैंसर, एनीमिया जैसी बीमारियों के बजाय एड्स पर हम अरबों रुपये की रकम हर साल फूंक रहे हैं. आख़िर क्यों? इस सवाल का जवाब तलाश रहे हैं टीवी पत्रकार दिलीप मंडल. उनका यह लेख आज ही नवभारत टाइम्स में छपा है. इयत्ता में उसे यथावत लिया जा रहा है दिलीप जी की अनुमति से.

एड्स और एचआईवी इनफेक्शन अजीब बीमारी है. इसके बीमार आपको शायद ही कहीं नजर आएंगे, लेकिन इससे लड़ने की कोशिश आपको हर जगह दिखेगी. कहीं सरकारी एजेंसियां पोस्टर बांट रही हैं, तो कहीं एनजीओ नुक्कड़ नाटक खेल रहे हैं तो कहीं कंडोम बांटने की मशीनें लगाई जा रही हैं तो कोई रेड लाइट एरिया में पर्चे बांट रहा है. इस कोशिश का असर ये है कि देश का बच्चा-बच्चा इस बीमारी का नाम जानता है, और ये बता सकता है कि ये एक जानलेवा बीमारी है. ये बात हमें इतनी बार और इतने तरीके से बताई जा रही है कि इससे अनजान रहना संभव ही नहीं है.
और ऐसी ही खतरनाक बीमारी से संक्रमित लोगों की संख्या एक साल में आधी से कम रह गई. इस साल जून महीने तक सरकार से लेकर एनजीओ तक हमें ये बताते रहे कि देश के हर सौ में एक आदमी एचआईवी पॉजिटिव है. अब देश के स्वास्थ्य मंत्री अम्बुमणि रामदॉस कह रहे हैं कि देश के 300 में से एक आदमी ही एचआईवी पॉजीटिव है. आंकड़ों के इस खेल का देश के ज्यादातर लोगों के लिए कोई मतलब नहीं है क्योंकि हम और आप अपने अनुभव से जानते हैं कि हमारे आस पास लोग कैंसर , टीबी, दिल के दौरे जैसी बीमारियों से मरते हैं, एड्स या एचआईवी इनफेक्शन से नहीं। लेकिन इन आंकड़ों से देश के कुछ हेल्थ प्रोफेशनल्स और एड्स पर काम करने वाले सारे एनजीओ एक सुर में हाय तौबा मचा रहे हैं. हंगामा ऐसा , मानों एचआईवी पॉजिटिव लोगों का आंकड़ा कम बताए जाने से आसमान फट पड़ेगा.
वैसे आसमान फटने की उनकी आशंका बेबुनियाद नहीं है. देश-विदेश की दवा कंपनियों से लेकर हजारों एनजीओ और स्वास्थ्य मंत्रालय में बैठे अधिकारियों के लिए ये एक शानदार रोजगार या फिर कहें व्यवसाय है. आखिर ये कोई ऐसी-वैसी बीमारी नहीं है. इस पर केंद्र सरकार हर साल एक हजार करोड़ रुपए से ज्यादा खर्च करती है. राज्य सरकारों का खर्च इससे अलग है. विदेशी फंडिंग एजेंसियों से सैकड़ों करोड़ रुपए इसके अलावा आ रहे हैं.
लेकिन ये रुपए आखिर आ क्यों रहे हैं? आखिर हर देसी-विदेशी एजेंसी इतनी दरियादिल क्यों बनी हुई है? इसके लिए ये जानना जरूरी है कि क्या ये बीमारी दरअसल उतनी खतरनाक है, जितना कि हल्ला मचा हुआ है. अगर आपकी जानकारी में - आपके मोहल्ले में, या आपके परिचित लोगो में दो-चार लोग भी इस बीमारी से मरे हैं, तो इस बारे में शक करने का कोई कारण नहीं है. अगर ऐसा नहीं है तो इस बात पर हम सबकों सवाल उठाना चाहिए कि सरकार जिस बीमारी पर हजारों करोड़ रुपए खर्च कर रही है, उसकी वजह क्या है.
सरकार कहती तो ये है कि एड्स खतरनाक बीमारी है. लेकिन सरकार के अपने आंकड़े इससे उल्टी तस्वीर पेश करते हैं. इस सिलसिले में आप 22 नवंबर 2006 को हुई लोकसभा की कार्रवाई के अंश लोकसभा की साइट पर देख सकते हैं. इस रोज एक सवाल के जवाब में केंद्रीय स्वास्थ्य मंत्री अंबुमनि रामदॉस ने बताया कि 1986 से लेकर अब तक देश में 10, 170 लोगों की मौत एड्स से हुई है. यानी हर साल लगभग 500 लोग एड्स से मरे हैं. जबकि देश में हर साल साढ़े तीन लाख से ज्यादा लोग टीबी का इलाज न हो पाने की वजह से दम तोड़ देते हैं। कैंसर से भी हर साल लगभग इतनी ही मौतें होती हैं. एड्स से मौत का जो आंकड़ा सरकार पेश कर रही है, उसमें साल दर साल कोई बढ़ोतरी नहीं हो रही है. लेकिन 2005-2006 के आंकड़ो को देखें तो सरकार ने एक साल में एड्स नियंत्रण पर 533 करोड़ रुपए खर्च किए, जबकि टीबी नियंत्रण पर 186 करोड़ और कैंसर नियंत्रण पर सिर्फ 69 करोड़ रुपए खर्च किए गए। एड्स नियंत्रण का एक साल का सिर्फ केंद्र सरकार का खर्च एक हजार करोड़ रुपए को पार कर चुका है.
आखिर इस पहली का राज क्या है? स्वास्थ्य क्षेत्र की ऐसी गुत्थी है, जिसका हम सबसे वास्ता है. इसके दो संभावित जवाब हो सकते हैं. या तो सरकार सचमुच समझती है कि एड्स एक गंभीर समस्या है और स्वास्थ्य बजट का सबसे बड़ा हिस्सा खर्च करना चाहिए। या फिर ये किसी षड्यंत्र का नतीजा है, जिसमें सरकार जाने या अनजाने में हिस्सा बन गई है. एड्स और एचआईवी को लेकर दुनिया भर में जो बहस चल रही है, उसमें एक आरोप लगता रहा है कि ये दवा कंपनियों का खेल है. इस बीमारी के फर्स्ट लाइन ट्रीटमेंट पर हर महीने 60 रुपए का खर्च आता है जबकि सेकेंड लाइन ट्रीटमेंट पर हर महीने 8,000 रुपए खर्च होते हैं. एड्स नियंत्रण के तीसरे चरण पर भारत सरकार 6806 करोड़ रुपए खर्च करेगी जिसका 17 फीसदी इलाज पर खर्च होगा. इस पैसे का बड़ा हिस्सा दवा कंपनियों के पास पहुंचेगा.
लेकिन इस रकम का बाकी हिस्सा लोगों में एड्स को लेकर जागरूकता बढ़ाने और बीमारी के नियंत्रण जैसे कामो पर खर्च होगा. इस काम में सरकार की मदद करेंगे देश के हजारों एनजीओ. दरअसल एड्स नियंत्रण के पूरे जंजाल में एनजीओ की भूमिका बेहद रोचक है. 23 अगस्त 2006 को लोकसभा में दिए गए जवाब में स्वास्थ्य मंत्री ने बताया कि 2005-2006 में सरकार ने 103 करोड़ रुपए एनजीओ को दिए. ये रकम वर्ल्ड बैंक, डीएफआईडी और यूएसएड से जुटाई गई. इसके अलावा बिल और मिलेंडा गेट्स फाउंडेशन ने 180 करोड़ रुपए एनजीओ को दिए. दूसरी अंतरराष्ट्रीय एजेसियों ने भी इसके अलावा 150 करोड़ रुपए से ज्यादा रकम एजीओ को दी. इस रकम का एक मामूली हिस्सा भी अगर गर्भवती महिलाओं को आयरन टेबलेट देने पर खर्च किया जाए तो हर साल कई लाख गर्भवती महिलाओं की मौत टाली जा सकती है. एड्स नियंत्रण में जुटी एनजीओ को मदद पहुंचाने में अंतरराष्ट्रीय एजेसिंयों के उत्साह का कारण समझ पाना मुश्किल है.
वैसे कई बुद्धिजीवी अरसे से ये कह रहे हैं कि दुनिया भर में एनजीओ तंत्र को बनाने और बढ़ाने के पीछे एक बड़ा षड्यंत्र है. इस बारे में कई रिपोर्ट और किताबें आ चुकी हैं. खासकर दक्षिण अमेरिका में एनजीओ की भूमिका पर काफी शोध हुआ है. भारतीय संदर्भ में एनजीओ में जुड़े लोगों की पृष्ठभूमि को देखें तो उसमें वामपंथी और समाजवादी पृष्ठभूमि के लोग ज्यादा हैं. कुछ लोग स्वदेशी आंदोलन वाले भी हैं. ऐसा ही लैटिन अमेरिका में भी हुआ है. लैटिन अमेरिका में ये आरोप लगते रहे हैं कि जो भी लोग बदलाव की बात करते हैं उनका नेतृत्त्व करने वालों के एक हिस्से को एनजीओ के दायरे में समेट लेने की कोशिश हो रही है, ताकि बदलाव की ताकतें कमजोर हों. क्या भारत में भी यही हो रहा है? ये सवाल इसलिए भी उठ रहा है क्योंकि अगर विदेशी एजेसिंयों की दिलचस्पी भारतीय नागरिकों की सेहत में होती तो ज्यादा रकम टीबी, कैंसर, मलेरिया और डायरिया कंट्रोल के लिए आती. इन बीमारियों के नाम पर एक एनजीओ बनाकर आप सरकार या विदेशी एजेंसियों से मदद लेने की कोशिश करें तो आपके लिए कोई भी अपनी थैली नहीं खोलेगा. लेकिन एड्स और एचआईवी के लिए पैसा देने को सरकार भी तैयार है और विदेशी एजेंसियां भी. बदलाव का हथियार बनने की क्षमता रखने वाले युवा दीवारों पर एड्स की जागरूकता के पोस्टर चिपकाएं, पर्चे बांटे, नुक्कड़ नाटक करें और कंडोम बांटे, ये हमारी सरकार को भी बुरा नहीं लग रहा होगा.

Prime focus is on strengthening condom access by increasing the number of outlets selling condoms to 30 lakhs, an increase from 6 lakhs at present.

    I am not an economist, so I will not comment on who makes what in profit.

    But as a pediatrician, I do know that it is a dangerous disease, far
    more serious than BP or TB, as HIV/AIDS disease has wiped out nations leaving only orphans behind. We cannot wait in India to catch it.

    Yes the numbers have been adjusted, but that is due to study in different population. Previous stats were from high risk groups like brothel workers, where higher concentration was seen. Current stats are from general community with much larger sample size, and may be indicating the true prevalence of our country. But that is not satisfactory answer, as much more needs to be done.

    Why?

    Because of illiteracy and awareness, and false hope that it cannot touch you.

    For example, in Bihar, 78% of deliveries happen at home, and almost 78% of women do not know that condom usage is one of the ways to prevent HIV/AIDS (National Family Health Survey 2006, same dataset which reduced HIV prevalence). And similarly a high proportion of young men go outside to be laborers or home maid in big metropolis, who bring HIV back to villages of Bihar. How comfortable will you be in hiring such an individual in your home, as there are no obvious signs of HIV for years? Same story is for truck drivers in different states.

    Also Note that sex is not the only way to transmit HIV. How about someone has a delivery or an accident and gets blood in hospital/private clinic/buy blood from a stranger, which is not checked for HIV. How about a small child playing in the backyard, and gets stuck with infected needle from IV drug user. How about pre-marital sex (nearly 40% kids under 18 years, as per NDTV, have had sex; even if inflated numbers but certainly the provocative influence of TV, cable, Mobile, and new bollywood strip-tease is there.

    Yes, we do not get shocked about the amount of money paid to actresses to show off on screen, but someone is feeling sorry for free treatment. That is pathetic!

    Why does West funds for HIV & AIDS, rather TB, malaria etc.

    That is not true, as Bill and Melinda Gates Foundation have huge funding for curable diseases like TB and Malaria globally. But more than that, they also have concerns of global spread of this disease, if not checked properly. And since there is no cure for HIV/AIDS, preventive education is the only tool we have for now to fight against this disease.

    The data may say you have 1 in 300 chance to get HIV, so what? For that individual and for that family, it is 100% doom.

    I think the answer should not be an objection to funding, rather active participation and use of Right to information act to show visibility of funding at all level, that the needy is not being trashed out, but rather gets what is meant for him, that someone shows that the testing and treating centers are functional, and not only on paper, and to local data of performance available to public. That is how a newspaper can help, and not diverting the attention to another irrelevant sphere.

    It is not for nothing, that I felt that there is not even a good website on HIV-AIDS in Hindi.
